We are proud to be an in-network dental provider for Delta Dental Plans and a few select Cigna Dental Plans. We always recommend that you call your insurance company to see how you can maximize your dental benefits at our office. We accept most dental insurances as an out-of-network dental provider, and as a courtesy to all of our patients, we will file all your insurance claims on your behalf. Unfortunately, we do not accept the State of Connecticut HUSKY Dental Insurance at this time.
Please make sure you give us all your current insurance information. We remind you that your insurance policy is an agreement between you and your insurance company, not between our office and your insurance company. If you have questions about coverage please call our office or contact your dental insurance with information provided for you on the back of your insurance card or provided by your employer. We can make no guarantee of any estimated coverage, but we’ll do our best to see that you receive your maximum benefits. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.

A prepayment courtesy of 5% will be subtracted from the total amount due if paid in full PRIOR to the first treatment date, and if payment in full is made by cash, check, or credit card.
For patients with insurance, the same 5% courtesy is offered as stated for those with no insurance; however the courtesy is based on the estimated patient’s portion ONLY, not any portion due from the insurance company. Payment must be made by cash, check, or credit card.
With or without insurance, you may choose to pay-as-you-go, paying your entire obligation for each visit at the time of the visit.
For single appointment treatment (x-rays, exams, cleanings, fillings) payment or co-payment is expected at the time of treatment – by check, cash, or credit card.
For multi-appointment treatment (crowns, bridges, implants, dentures), 50% of the payment or co-payment is due at the first visit, with the balance due at the second visit.
If you are 63 or older, and you are not covered by dental insurance, we offer a 10% courtesy on all procedures, when payment is made in full by cash, check, or credit card at the time of treatment.
– For single appointment treatment, full payment is due at the end of the appointment.
– For multiple appointment treatment, 50% is due at the first visit, and the balance is due at the second visit.
The 60-Day Three-Payment option is available to patients paying by credit card whose treatment plans exceed $1,500.
The patient signs a credit card authorization form, allowing us to make the following charges to their Visa, MasterCard or American Express:
– 33 % of the total due is charged to your card at the start of treatment
– 33 % is charged 30 days later
– The remaining 33 % is charged 30 days after 2nd payment.
Lumos Dental offers third-party financing through CareCredit, a healthcare patient payment program. The program is available to anyone, regardless of insurance status. The application is very quick and is completed in-office. You will know within minutes if you qualify.
